I'm taking this to Tristan's appointment this week.  Hopefully it will get me some results….




Re: Tristan E. Davis, Patient IHC Pediatric Rehabilitation
        Parental Concerns

Diagnosis
            -Congenital Muscular Torticollis
            -Pseudotumor R side sternocleidomastoid
            -L side plagiocephaly

Original Tx referral
            -Physical therapy

Outcome
            -Greater occurrence of regression to torticollis position
            -Short periods of improvement
            -Intermittent difficulties with breastfeeding due to positioning
            -Failure to achieve certain developmental milestones:
                        *Lifting head and upper body in prone position
                        *Rolling from back to stomach and visa versa
                        *Holding head up for age appropriate amount of time
            -Bossing of L forehead
            -Flattening  L anterior head
            -Bossing R temporal region

Tx options to consider
-Helmet to correct plagiocephaly, planned for 3 mo with Northwest Orthodics
            -TOT collar to assist in holding up head
            -Botulinum injections/baclofen po to relieve tightness

Perhaps an MRI would be warranted at this time to further evaluate the condition of the sternocleidomastoid muscle and rule out any other structural abnormalities
